What RxFolio is
RxFolio is a personal health-information management and medication-tracking tool. It helps you record, organize, and retrieve information that you choose to enter or upload — medications, schedules, vitals, symptoms, conditions, allergies, procedures, documents, and related notes.
What RxFolio is not
RxFolio is not, and does not claim to be:
- A doctor, hospital, clinic, pharmacist, or healthcare professional;
- A medical device or a regulated clinical decision-support system;
- An emergency service or a monitoring service;
- A source of medical diagnosis, treatment, or prescription;
- A substitute for professional medical advice, examination, or care.
RxFolio does not diagnose disease, prescribe treatment, recommend that you start, stop, or change any medication, guarantee medical outcomes, or replace consultation with a qualified clinician.
Information may be incomplete or wrong
Information displayed by RxFolio may be incomplete, inaccurate, outdated, incorrectly entered, incorrectly interpreted, or affected by technical problems. This includes:
- Information you or another person typed into the application;
- Information recorded against a prescription, report, bill, or discharge summary;
- Summaries, trends, and charts generated from your records;
- Visit-preparation reports, whose wording is drafted automatically.
Always verify important medical information against the original prescription, report, medical record, or with your healthcare professional before acting on it.
Reminders are organizational aids only
Medication reminders and notifications are convenience features. They can fail or arrive late because of device settings, operating-system restrictions, battery optimization, missing notification permissions, connectivity problems, application state, device failure, or service outages. Do not rely on RxFolio as your only mechanism for taking medication on time.
Visit-preparation reports are educational only
A visit-preparation report is assembled from the records you select, and its wording is drafted automatically. The result is informational and educational: it is not medical advice, not a diagnosis, and not dosing authority. Automatically generated text can omit something important or state something confidently and incorrectly. Review every report against your own records before you rely on it or share it, and treat it as a starting point for a conversation with a clinician, never as a conclusion.
Your responsibilities
You remain responsible for:
- Taking medications as directed by your healthcare professional;
- Confirming medication names, strengths, and dosages;
- Following prescriptions and treatment plans;
- Attending medical appointments;
- Obtaining professional medical advice;
- Responding appropriately to medical symptoms or emergencies;
- Reviewing any report before sharing it with a healthcare professional.
Caregivers and profiles for other people
If you use RxFolio to record information about another person, such as a parent, child, or dependent, you are responsible for having the authority to do so and for the accuracy of what you record. RxFolio does not verify clinical accuracy, authority, or consent on your behalf.
